USA Hockey Registration Instructions

This year the Eagles will be joining the thousands of other hockey players/clubs who complete their USA Hockey "Individual Member Registration - IMR" forms and "Waiver of Liability" forms and pay their USA Hockey and affiliate (MO Hockey) fees on-line.  If you do NOT have access to the internet to register on-line, please notify Registrar Barb Tomblinson ASAP at btomblinson@earthlink.net.
 
Go to www.usahockey.com and follow the on-line registration instructions to register EACH PLAYER & COACH in your family.
 
On-Line Registration Notes: 

1. To get to the registration for players and coaches, click on "Players" on the USA Hockey home page, top left side.

2. Be sure to register for the 2008-2009 season and be sure to register for MO (Missouri). 

3.  Please proofread the data you enter carefully, especially birthdates.  Remember:  garbage in; garbage out!

4.  Have your credit card handy.  The USA Hockey Fees are - $37/player/coach (includes MO Hockey Affliiate Fee of $7)

5.  Print a hard copy of the confirmation page.  A copy is also e-mailed from USA Hockey to you as an adobe.pdf file that can be saved on your hard drive.  If the form is lost, a duplicate can be provided by logging onto www.usahockey.com.

6.  Provide the Eagles Registrar with a copy of your confirmation page(s) either in person or via eamail by Sept. 15 .  You aren't officially "registered" until the Eagles registrar inputs your confirmation number to the club's CyberSport software and the registrar's computer data base is updated on-line by USA Hockey.



7 .  The Eagles will still need to manually complete and keep on file the USA Hockey "Consent to Treat & Medical History" and "Player Code of Conduct" forms.
