Jeff City Parks & Recreation Registration Information

 

A fully completed and signed Parks and Recreation Registration Form for each individual player on the roster must be submitted with each team roster, along with a personal check for $25 PER PLAYER made payable to Jefferson City Parks and Recreation.  The fee is non-refundable.  However, it will go into the respective club's account and be applied to their first month's ice invoice. 

Please complete the form for EACH of your players, along with a check for $25 for EACH player made out to Jefferson City Parks & Recreation.   Mail the form & check to Barb Tomblinson, Treasurer, at Eagles Hockey, P.O. Box 2264, Jefferson City, MO  65102.  DO NOT MAIL THE FORM & CHECK TO PARKS & REC.!!!
 
Forms for the 2008-09 season need to be received by July 31, so please act now!
